Integrated Home Care Service |
UCNCHS established Home Help Service Team on November 1, 1998 and Home Care Service Team in December 1999. Starting from April 1, 2003, home help service was integrated with home care and meal services to become the new Integrated Home Care Service Team with the aim to provide quality home care service to residents in Tai Po.

Our rate is calculated with reference to the standard of Social Welfare Department, and the economic status of the applicant. |

Service Hours |

Monday to Saturday, 9:00 a.m. to 6:00 p.m. Service will also be available on Sundays and public holidays if necessary. |

Eligibility |
| |
|
Applicants should live in the districts we are serving. |
| |
|
People lack relative and social supports, low income group, and ones need urgent home care service will have a higher priority. |
| |
|
How to Apply |
| |
|
Applicants can contact our staff in the service centre. We also consider referral from hospitals, social welfare department or other government departments. |
| |
|
Once the application is received, our social workers will conduct home visit to assess the eligibility of the applicants. They will also tailor-make the type and frequency of services which can suit them most. |

Quit/
Terminate Service |
| |
|
Clients can quit and terminate service anytime. |
| |
|
Our social workers will conduct evaluation periodically. Clients will be informed in advance if they are suggested to terminate service. Besides, our social workers can refer clients to other service. |
